Many muds these days have begun to do this more often. Have a system of randomized items, very similar to diablo system of trash items, mediocre items, magical items, rare items, etc., this allows us to have many more items easily, and allows builders to build very unique items.
Loot tables are good too or at least some rules around what stuff you can get. There's no reason for a boss/dungeon/major enemy to spawn trash items. On the other hand, if skeletons are using swords they probably aren't using nice, shiny ones unless some necromancer is feeling particularly detail oriented. I think rare should be below magical. Magical/enchanted stuff should generally be better than very good craftmanship. Although I suppose it could be a parallel attribute so that you could have trash magic items and mediocre magic items, etc.
